Responsibilities
- Assess member needs and support care coordination across clinical, behavioral health, and social service resources
- Document care plans, referrals, outreach, and member progress according to program requirements
- Coordinate with providers, facilities, families, and community partners to support member outcomes
- Follow Medicaid, LTSS, clinical, quality, and utilization management guidelines where applicable
Requirements
- Care management, clinical review, nursing, behavioral health, social work, or member support experience
- Relevant license or degree if required by the employer posting
- Experience serving Medicaid, Medicare, LTSS, dual eligible, or managed care populations preferred
- Strong documentation, communication, and member advocacy skills
