Responsibilities

• - Conduct face-to-face program assessments
• - Support person-centered planning and care coordination
• - Coordinate LTSS, IDD waiver, behavioral health, and physical health needs
• - Serve as a single point of contact for members with complex needs
• - Document short-term and long-term service goals
• - Support utilization and authorization requests
• - Report critical incidents to appropriate parties
• - Assist with appeals, fair hearings, grievances, and state audits

Requirements

• BA/BS degree required
• - Minimum 2 years of experience working with a social work agency
• - Equivalent education and experience may be considered
• - Healthcare-related field of study preferred
• - Ability to travel to worksites and other locations as needed
• - Experience with LTSS, IDD, care coordination, or Medicaid waiver programs preferred
